What Information We Collect
To obtain a library card, we ask for and record the following information on paper and in an electronic database: full name, address, phone number, work address, work phone number, birth date, gender, PIN, and e-mail address.
A PIN (Personal Identification Number), in conjunction with the library card number indicated on your card, is required to access the online reserves and view your account information from one of the library’s OPACs (computer catalogs) or from any computer with Internet access when you use our WebPac (web catalog). We do not write your PIN on your card; it is your responsibility to protect your PIN. Your PIN may be changed by staff upon request and is confidential. If you lose your library card, please notify us as soon as possible.
Confidentiality of Library Records
By Indiana Code IC 5-14-3-4, and American Library Association policies on confidential records, all records relating to library patrons and their use of library materials and services are strictly confidential. Library staff members are not authorized to disclose such records to any third party. Minors are accorded the same confidentiality rights as adults under Indiana law.
We do not make any information about you or your use of library services or materials available to anyone or any group except in these situations:
Subpoena, Search Warrant, Court Order
We reserve the right to cooperate with law enforcement personnel to provide information when requested. When presented with a subpoena, search warrant, or court order, the library may consult legal counsel to assure the document is in proper form before complying.
Emergencies involving public health/safety may require an immediate decision. In such cases, the Director or other designated staff member will be contacted. Legal counsel will be consulted when possible.
Parent/Guardian Wants Information
The laws of Indiana require a library to provide information about a minor child to her/his parent, guardian, or custodian.
USA Patriot Act
The 2001 USA Patriot Act expands federal law enforcement’s surveillance, seizure and investigative powers. If library records are requested under the USA Patriot Act, the law states that, in certain circumstances, library staff cannot inform the person about whom the information is requested, cannot speak to co-workers, the media or other government officials about the inquiry. Such requests may only be reported to the appropriate higher authority within the library.
In brief, under the USA PATRIOT Act, federal agents may request records of your library activities (including materials you borrow and your computer workstation usage). That federal law prohibits Library staff from informing you or anyone else if federal agents have obtained records about you.
Internet Session Data
To use an Internet computer at the Crawfordsville District Public Library, you must be at least 10 years old. We do not save the history files for web-browsing sessions, and the recently visited URL list available in the web browser is deleted after your session is over (when you close the browser).
You log on to an Internet computer with your library barcode/password or your guest pass barcode/password. The history of your log on is retained but no attempt is made to match a barcode with a patron unless the library staff suspects misuse of a card or computer (e.g., patron using stolen card, patron using borrowed card, patron vandalizing computer, etc.).
It is the patron’s responsibility to log out of any web site service or e-mail account when finished.
Our Internet Use Policy was adopted by the Library Board on June 14, 2000. Patrons who use one of our Internet terminals agree to this policy when beginning a web session in order to proceed.

Library Web Site
We attempt to provide timely and accurate information on our web site, but there is always a chance that information obtained through our web pages may be incorrect or not current. We will make an effort to correct errors brought to our attention. Any person or entity that relies on any information obtained from this system does so at his or her own risk. The Library shall not be liable for any losses caused by reliance on information obtained on the Internet.
Our web site contains links to external web sites and databases. The Library is not responsible for the privacy practices, security, or content of these external sites. We recommend that you familiarize yourself with the privacy and security information for any site you visit. We do not guarantee the accuracy, relevance, timeliness, or completeness of any outside information. The use of these links does not reflect their importance, nor is it intended to endorse any views expressed or products or services offered by these other web sites.
If you disclose personally identifiable information or personally sensitive data through other web sites, chat rooms, online communities, or other public online forums, this information may be collected and used by others.
Two types of “cookies” are commonly in use on the web:persistent cookies and non-persistent cookies.
A persistent cookie is information that a web site puts on a computer’s hard drive so that it can remember something about the user later (provided that the user is on the same computer). The main purpose of a persistent cookie is to identify users and possibly prepare customized web pages. Persistent cookies are programmed to expire after a certain time. When a cookie expires, the computer discards from memory and it is no longer saved. The Crawfordsville District Public Library does not use persistent cookies on its web site.
A non-persistent cookie (a “session” cookie) is information that a web site puts on a computer’s temporary memory that keeps track of a user’s activity on that site. Non-persistent cookies are temporary and are discarded once a user’s session is completed. The Crawfordsville District Public Library does not use non-persistent cookies on its web site but it does use them on itsWebPac (our online catalog). Each time you access the WebPac, you will be given several non-persistent cookies that will remain with you throughout your WebPac session but which will not remain after the session. These cookies simply hold your account information (if you log onto your account) while you use the online catalog.
Our commitment to protecting the privacy of the young is important to us (our patrons must be at least 10 years to use an Internet computer). We urge that parents or guardians supervise their children’s use of the Internet, especially when a child wishes to give out personal information online.
Miscellaneous Library Transactions
The Library cannot guarantee that transactions over the Internet, by telephone, or at a public service desk are secure and confidential. We make every effort to protect our system and servers from unauthorized access, but the Library may be vulnerable to temporary breaches of security. You must choose the method you wish to communicate your requests and personally identifiable information to us.
